The forms below are an important step in the completion of your requirements to work at camp this summer. Once you have received your contract, the forms will be available to you. Some forms are filled out entirely online and some need to be printed and uploaded to your CampInTouch account. Each uploaded form will need to be submitted as a single PDF document (the system will not allow you to upload additional pages ). CamScanner (an app for your phone) and pdfjoiner.com are free digital tools that can help you scan and join PDF documents.
All forms are due May 2nd, 2022.

To complete your forms (once they are available), please follow these steps:
- Click this link to reach the CampInTouch log-in page
- Log in to your CampInTouch account. Use the password that you created when you filled out your application. (If you do not remember your password, click the link “retrieve/set password” on the bottom-right hand corner of the log-in box.)
- Once logged in, click on the link “Forms and Documents.” You will see links to the forms you need to fill out.
Health History
This form can be filled out completely online. When you are finished, click “submit.”
Insurance and Authorization Form
This form will ask for copies of your Health Insurance card (front and back) and your Prescription Card (front and back) if you have one. You can copy them, tape them onto the form, and be sure to sign at the bottom. Then scan and upload this form into your CampInTouch account.
Medical and Imm. Form - Filled by Doctor
This form should be filled out by a physician. We will also accept a state form or printout from the doctor with the same information. Wisconsin State Law requires that the exam be within 2 years of attendance at camp, so we will accept a form based on a physical given August 11, 2019 or later.
