Complete online forms
We do NOT accept mailed or paper forms. They must be submitted electronically either through CampInTouch or Campanion. If you have questions on how to do this, please review these instructions. If you have additional questions, please contact the GUCI office at guci@urj.org.
Most of the forms we need before your camper can come to camp are completed online. You can login into the CampInTouch system at any time to check the status of your account, make sure your contact information is up-to-date, and to complete the pre-summer forms.
All forms necessary for your camper(s) will be listed on your Forms Dashboard within your account. Please note that if you stop in the middle of completing one of the forms, what you have already entered will NOT be saved. It is important that you be prepared in advance to complete the forms.
Once you have submitted a form, you can review the information that you entered but you cannot edit it. You will have to get in touch with us if you want to change the information. For example, if you signed up for a Bus home and you want to cancel it or change to a different bus, please send an email to guci@urj.org for additional instructions.
Camper Information Form
Must be completed for each individual child attending. This is information that our team needs to prepare for your camper’s arrival.
Transportation Form
Must be completed for each individual child attending. If your camper is flying to or from camp and will need transportation, please contact the office in addition to filling out this form.
Health and Medical Forms
Must be completed for each individual child attending. The Health Forms are a combination of online forms as well as forms you will need to download, print, complete and upload into your account.
Please note we do not accept paper forms – all forms must be submitted electronically. If you need any assistance with this, please contact the office.
Insurance Cards
Please upload copies of your insurance card to your CampInTouch account or Campanion account.
Medical Authorization
Please print and sign the authorization box before uploading to your CampInTouch account.
B'nai Mitzvah Form
ONLY for campers whose bar/bat-mitzvah is within six months of camp.
